ZERTIVA® IS GEARED TOWARD PATIENTS WITH CHRONIC CARDIAC INSUFFICIENCY AND CORONARY HEART FAILURE
Our patients receive a scale, a blood pressure monitor, and a modem. Their vitals are transmitted to the telemedicine center on a scheduled, daily basis and the data will automatically be recorded in the patient’s electronic file. If the values deviate from the patient’s individual thresholds, an alarm will be triggered and the patient will be contacted by the medical providers to clarify the cause. Furthermore, patients can expect an immediate response in case of weight gain caused by water retention that occurs when medication is taken irregularly. Additionally, the patients within the Zertiva® program are regularly contacted and surveyed. Training courses about nutrition, exercise, and pharmacotherapy complete the program.

The Zertiva® telemonitoring program offers an invariable sense of security that comes with the consistent supervision and treatment of coronary heart failure. Your vital signs are uninterruptedly recorded and transferred to the SHL Telemedicine center; these vital signs include your blood pressure, body weight, and potentially your ECG. There are several stages of treatments depending on the degree and circumstance of the disease. These include conversations over the phone and regular vital sign checkups through data transfer over the phone.
WHO DOES THE REGULAR MEASUREMENTS?
You will receive the necessary measurement devices such as the blood pressure monitor at no extra cost. Our devices are designed to be very user friendly and the data transfers are very straightforward. If the values deviate from your individual thresholds, the telemedicine center will immediately contact you (and your physician, if necessary), so that appropriate countermeasures can be taken.
Each patient receives a customized telemedicine patient card which instantly provides the doctor with an authorized point of contact from SHL Telemedicine in case of an emergency.
